INDEPENDENT CONTRACTOR COURIERS!

The following are the minimum requirements for contracting with Phoenix Courier:

• DBA, Fictitious Name or formalized corporate structure
  
• Federal Tax ID number
  
• Business Checking Account
  
• Business Card
  
• Contractor must provide a 3 year driving record—( www.dot.state.pa.us )
  
• Contractor must provide copy of valid driver’s license
  
• Contractor must provide copy of valid vehicle registration
  
• Contractor must provide copy of valid insurance card
  
• Contractor must provide copy of valid insurance declaration page
  
• Minimum vehicle insurance coverage in the amounts of $100K per person liability-$300K per occurrence--$50K property damage
  
• Worker’s Compensation Insurance or Occupational Accident Coverage through Association for Delivery Drivers-(www.a4dd.org)
  
• Register with Contractor Management Service—process settlement check ( www.enrollisp.com )
  
• Completion of Independent Contractor agreement
